Assessment Options
Dr Schultz is able to facilitate both in-person and online Zoom assessments. In person assessments are conducted by mutual agreement after reviewing the documentation.
In-person assessments are conducted at Dr Schultz’s office at The Commons, Level 1, 55 Collins St, Melbourne 3000.
Engagement Terms
Making a Booking
Assessment bookings are made by contacting Dr Schultz's office via email or the contact form on this site. Independent Medical Examinations must be booked by the examinee's legal representative.
Each booking constitutes an offer to purchase medicolegal services from Dr Helen Schultz and her associated entities under these published terms. Dr Schultz reserves the right to accept or decline bookings at her discretion.
Booking Fee
A booking fee of 10% of the total assessment fee secures your appointment and is invoiced at the time of booking, payable within 3 business days. This fee allows Dr Schultz to set aside dedicated time to review all accompanying documentation and prepare appropriately for your client's assessment.
The booking fee is credited against the final invoice and is refundable only with at least 10 days' notice of cancellation.
Changing or Cancelling Appointments
Bookings may be cancelled at any time by providing written notice via email.
Cancellations made with 10 days' notice or more attract no cancellation fees. Dr Schultz is happy to accommodate date changes or process refunds of booking fees paid in these circumstances.
Cancellations made within 10 days of the scheduled assessment attract a cancellation fee of 20% of the total assessment cost.
Cancellations made within 48 hours of the scheduled assessment attract a cancellation fee of 40% of the total assessment cost.
Non-attendance or failure to arrive for a scheduled assessment without notice attracts a fee of 50% of the total assessment cost.
Assessment Documentation
Assessments can be conducted in person or online. In-person assessments are conducted by mutual agreement after reviewing the documentation.
A formal Letter of Instruction and all supporting examinee material must be provided electronically at least 7 days before the scheduled assessment date.
Dr Schultz does not accept hard copy materials.
Failure to provide complete documentation within this timeframe will result in cancellation of the assessment, with applicable cancellation fees payable.
Payment Terms
Tax invoices are issued following completion of each assessment, with payment required within 7 days. Prompt payment is appreciated.
A late payment fee of $50 applies to invoices remaining outstanding beyond 7 days.
Report Delivery
Completed reports are provided within 10 business days of assessment.
Where urgent turnaround is required, Dr Schultz may accommodate expedited delivery at her discretion for a rush fee of $420 + GST.
Reports are released upon receipt of full payment.
